After the accident

Three temporary loaders at a freight yard were arranged to unload hazardous chemicals sulfuric acid. According to the procedure told by the commander, they first connected the upper discharge pipe of the tanker with the flange of the transfer pipe and pressurize the tank. When the pressure reached the required level, the sulfuric acid still did not flow out, so they took out the decompression to open the slot cover and inspected it and found that the discharge pipe in the tank was blocked. So three people arbitrarily dismantled the flange without consulting the commander, and used a steel pipe to insert the discharge pipe to clear it. A white foamy liquid up to 3 meters in diameter can be sprayed in the tube when the discharge tube is opened, and the body and the face of the three people standing on the tank are splashed with sulfuric acid. As the three people did not wear protective masks, the liquid entered the eyes of the three persons and was sent to the hospital after washing with water. The examination was for alkali damage.

Cause Analysis

1, the person responsible for unloading sulphuric acid is not completely cleaned sulphuric acid tank. Afterwards, it was learned that the sulphuric acid tank was previously used to contain liquid caustic soda. Although the sulphuric acid tank was cleaned, the discharge pipe on the tank was not cleaned in place, and the attached liquid caustic soda was clogged in the lower part of the discharge pipe. Pressure, so that the alkali, the reaction of salt water, acid, etc. have been sprayed.

2. The responsible stevedores are all temporary workers. They have poor safety awareness. They do not wear protective masks according to regulations and are not protected from dangers.

3. The responsible personnel of the freight yard are not supervised strictly and found that the unsafe practices of the temporary workers have not been stopped in time.
